On the 'E106 form' you can indicate the fact that family members are co-insured. In general, spouses and children up to a certain age are regarded as being co-insured as long as they do not have an independent right to insurance in their country of residence. Please note: some countries (such as Germany) do not accept your cohabiting yet unmarried partner as a co-insured.
Content of insurance for family members
Co-insured family members are entitled to medical care in accordance with the legislation of the country of residence. As such, claims may differ from the Dutch insurance package. Furthermore, for certain care you may have to make a personal contribution in the country of residence. More information about the procedures and reimbursements can be obtained from an insurer in your country of residence.
Co-insured family members have to pay premiums
Co-insured family members over 18 years of age have to pay premiums, the so-called Zvw (healthcare insurance) contribution. As the costs for medical care differ per country, the extent of this contribution depends on the country of residence. The contribution for each treaty country is stipulated on an annual basis.
The CVZ (Health Insurance Board) sends you a separate annual invoice for your co-insured family members. The invoice states the amount due for the year in question, plus the payment terms.
Calculation of healthcare insurance contribution
Each year, the Minister of Public Health stipulates the amount of the estimated average premium for healthcare insurance. This is referred to as the 'flat-rate element'. The healthcare insurance contribution is calculated by multiplying the flat-rate element by the country of residence factor. The country of residence factor is the sum of the average costs in a treaty country, divided by the average costs per insured in the Netherlands.

Changes to the family composition
Has the composition of your family changed following a birth or death for instance? Or does someone in your family gain an independent right to insurance by virtue of a job? Please report such changes to the insurer in your country of residence. They will assess whether a family member still meets the conditions to be co-insured with you.
If a person can no longer be co-insured, the insurer will report this to the CVZ, who will subsequently take the family member off the policy. You no longer will have to pay the CVZ the healthcare insurance contribution for this person.
It is wise to also notify the CVZ itself about any changes to the composition of your family. Please indicate if (and if so, when) the insurer in the country of residence was informed.
