When your care provider sends the invoice straight to you instead of HollandZorg, you can claim the invoice from us.
How does this work?
First you complete the claim form. The form itself will give you more information. Once your claim has been received, we will send you a new form which you can use next time.
Please submit the original invoices along with the claim form.
