H4i-Digital provides GP care for HollandZorg policyholders You can quickly get medical advice or contact a general practitioner (GP) in your own language via your phone or computer. If necessary, H4i-Digital can help you with a prescription, a follow-up consultation or an in-person appointment in Venlo or Tilburg.

If you already have a GP, you should contact them. If you want to use H4i-Digital, you must first deregister with your own GP.

That happens automatically - you don’t need to do anything.

You don’t need to have signed up as a patient with H4i-Digital, but you must complete the registration form. You can do this easily via the website. You will need the following:

• Citizen service number (BSN).
• Name and address details
• Date of birth
• Phone number
• E-mail address

No. Please have your details to hand when registering. It’s also useful if you know:

• What medicines you are taking
• Whether you have had any symptoms before

Go to the website and answer a few short questions about your symptoms. If you need to contact the GP, you can then call or send an e-mail immediately. The H4i-Digital contact details will then be displayed.

No. You do not need a DigiD to contact the GP at H4i-Digital.

Once you have answered a few short questions about your symptoms on the H4i-Digital website, you will receive advice straight away. If you urgently need to contact a GP, you can call immediately. You will then receive help as soon as possible, or you can choose a time to be called back. You may also send an e-mail. Describe your symptoms and you will receive a reply within 1 hour.

H4i-Digital can be accessed on weekdays from 8.00 am to 5.00 pm. At the weekend or outside opening hours, please call your local out-of-hours GP service in case of an emergency.

If your symptoms cannot be assessed over the phone or by e-mail and a doctor needs to examine you.

If the H4i-Digital GP considers that an in-person consultation is necessary, they will arrange an appointment with you at one of the H4i-Digital centres. These are now located in Venlo and Tilburg. If that doesn’t work for you, H4i-Digital will check with you what might be possible nearer to home.

No. If you are insured with HollandZorg, you will not be charged for GP care provided by H4i-Digital.

No. GP care is covered by your insurance, so you don’t have to pay anything for it. If H4i-Digital prescribes medication or refers you to hospital, you should bear in mind that you may be required to pay the personal contribution and/or compulsory excess. If you are insured with HollandZorg through your employment agency, you are covered for the costs of the compulsory excess and the personal contribution for medicines under the Flexpolis No Risk I and II policies.

H4i-Digital will arrange the referral. The hospital decides how the appointment is made. Where necessary, H4i-Digital will help you with this.
